Methods
The DFCI bone marrow transplant patient database was used to identify a cohort of patients with relapsed metastatic GCT of gonadal and extra-gonadal primary who were consecutively-treated with HDCT between 1997 and 2017 (n = 37). Patients treated with HDCT as first-line therapy for metastatic GCT were excluded (n = 2). The study population for this analysis thus included 35 individual patients.
Demographics, cancer features (primary site, primary histology, AJCC stage, IGCCC risk category [International Germ Cell Consensus Classification]), first-line treatment (chemotherapy type and cycle, response, post-chemotherapy surgery) and relapse (date, tumor markers and sites of metastasis) variables were collected from the electronic medical record. We categorized HDCT type into oxazaphosporine-containing (O-C) and CE regimens. We collected data to calculate Einhorn risk score (defined as 2 points each for IGCCCG high risk stage and platinum refractoriness; 3 points for third-line of subsequent chemotherapy) and Beyer risk score (defined as 1 point for platinum-refractory or absolute platinum-refractory disease; 2 points each for primary mediastinal tumor and for hCG level ≥1000 IU/liter).
Platinum refractory was defined as radiological, biochemical and/or clinical progression within 4 weeks of treatment with cisplatin prior to transplantation. Absolutely-refractory was no response to initial platinum-based chemotherapy5 . Salvage chemotherapy, HDCT and transplantation variables were detailed, including stem cell mobilization and collection metrics. Our primary outcome of interest was relapse after aSCT, defined as unequivocal disease progression evidenced by rising tumor markers, radiological and/or clinical progression.Dates of relapse, death and last follow-up were captured at the completion of the follow-up period (31 Dec 2017). Patient and treatment characteristics were summarized descriptively. We utilized Cox proportional hazards regression models to calculate hazard ratios and 95% confidence intervals (95% CIs) to estimate the univariable association between HDCT regimen and relapse and overall survival.Similarly, we also evaluated the univariable association between other clinical characteristics, including histology, IGCCCG, Einhorn and Beyer risk categories, platinum sensitivity, and line of therapy, and outcomes of relapse and death. We estimated overall survival by Einhorn risk category, HDCT early life infections regimen, and number of HDCT cycles using Kaplan-Meier curves. All p-values were two-sided with significance level of 0.05. All statistical analyses were completed using SAS v 9.4. The study was approved by the Institutional Review Board at the Dana-Farber Cancer Institute.
Results
Patient Characteristics
Baseline patient and treatment characteristics are summarized in Table 1. Twenty-nine (83%) testicular GCT and 6 (17%) extragonadal GCT patients underwent transplantation. The majority of patients had non-seminomatous primary histology (74%) and received HDCT as second line therapy (first salvage systemic therapy) (86%) and the remaining 14% received HDCT as third-line treatment. The IGCCC risk classification at time of first metastatic disease was 31% low, 26% intermediate and 40% high. At relapse prior to HDCT/aSCT, 77% of patients were platinum-sensitive and 23% were platinum- refractory. Twenty-one (60%) patients had non-pulmonary visceral metastases. All but one patient received bridging platinum chemotherapy prior to stem cell mobilization (median number of cycles: 2; range: 1 to 4) and TIP (paclitaxel, ifosfamide, cisplatin) was the most commonly administered regimen (60%) followed by VeIP (vinblastine, ifosfamide, cisplatin) in 17%. Distribution of Einhorn risk category at time of HDCT was 40% low, 43% intermediate and 17% high.
Stem cell mobilization and collection
Successful peripheral blood stem cell collection was achieved in 32 (91%) patients and 3 underwent bone marrow harvest. No TIP-treated patients who received one transplant only did so due to inadequate cells for a second transplant.
High-dose chemotherapy and autologous transplantation

Toxicity and treatment-related mortality (TRM)
